Employee Benefits Summary
Full-time - $30/month
Part-time - Pro-rata cost
Cost for coverage varies according to number of dependents.
Full-time and part-time employees are eligible, 100% employer paid benefit.
Life insurance benefit pays beneficiaries 6x annual salary or 12x annual
salary if accidental death occurs.
Long Term Disability
Full-time and part-time employees are eligible for this 100% employer
paid benefit. Employee receives 60% salary replacement.
Long Term Sick Plan
Full-time and part-time employees are eligible to accrue 2.15 hours per
80 hour pay period. This plan is used in conjunction with Worker’s
Compensation and State Disability.
Paid Time Off (PTO)
Full-time and part-time employees are eligible for this benefit which

25 Days per Year
This benefit is accrued at 7.69 hours per 80 hour pay period. Will result
in accrual of fewer days for employees working less than an 80 hour pay
Employees are allowed to take up to 3 days paid leave in the event of
an immediate family member’s death.
Full-time and part-time employees with one year of service are eligible
for up to $1,500 per year for continuing education.
Alternate Social Security
This is a very unique plan. All employees are enrolled, upon hire, as
an alternative to the Federal Social Security Program. Rates are:
- 6.05% Employee Contribution
- 3.25% Employer Contribution
All full-time and part-time employees are eligible upon hire
for this program. Up to 25% of your annual income can be tax deferred
in a variety of mutual funds, maximum of $15,000 per year.
After one year of service you may elect to participate in the Employer
Money Purchase Plan using the following scale.
1 - 5 Years (in the plan): 4%
6 - 10 Years (in the plan): 6%
10+ Years (in the plan): 8%
1 - 5 Years (in the plan): 1%
6 - 10 Years (in the plan): 4%
10+ Years (in the plan): 6%

Full-time and part-time employees are eligible to participate upon hire.
Flexible spending accounts are a great way to reduce your tax liability.

All full-time employees of the hospital receive a discount
in membership to join Evolutions Fitness and Wellness center.
Single: $28 (month)

All employees are eligible for a discount on some cafeteria items. Cafeteria
food may be charged and deducted from your paycheck every pay period.
All employees are eligible to participate. Bi-weekly dues of $1 allows
employee to attend the annual picnic and annual holiday party at no
cost (discount for family members) plus many more fun events throughout
Employee Assistance Program
All employees are eligible to utilize on-site Employee Assistance Program
services for themselves and their families.
